Sample Plan

Along their journey to living healthy for life, all our members start with a Strategic Plan for Health. The plan begins with baseline observations made at the initial assessment. Each member’s plan includes:

  • Initial health measurements along with space to update them (and show progress) at three at six months
  • Workout log to track your activities and heart rate
  • Food diary to keep track of your caloric intake
  • Nutritional information to help you make good decisions

Your Strategic Plan will become your workbook for better health as you begin your journey with Heuser. Bring it with you to every appointment, every workout, and keep it nearby at home to track your progress and encourage you on to better health.
